Living Will - $49

A Living Will is a document that discusses your preferences of prolonging/ not prolonging your life by extraordinary means, by artificial nutrition or hydration (if your condition is determined to be terminal or if you would be diagnosed as being in a permanently unconscious state).

This form will take approximately 15-20 minutes to fill out. Please make sure that all the information entered is correct.

What is a living will?

Hold up! Before you get ready to launch make sure you are on the right page. It can be confusing the difference between a Last Will and Testament , Health Care Power of Attorney, and a Living Will.

What does a Living Will do?

A Living Will give instructions to doctors and health care providers as to what to do with your personal health and wellbeing if you can’t make decisions for yourself or your Durable Health Care Power Of Attorney is not available. A Living Will does not have anything to do with transferring your property to the next generation. I only gives instruction to health care providers what to do if you can’t tell them how you want to be treated.

What do you need to be thinking about as you fill out this form?

Do you want the doctors to take “extraordinary” measures to keep you alive, including giving you hydration, nutrition, artificial breathing, and more. Or do you not want them to take “extraordinary” measures to keep you alive and you don’t want them to give you hydration, nutrition, or hook you up to machines.
